In a long-term cohort study published in Scoliosis (2015), researchers followed 34 adults with idiopathic scoliosis (average age 38, mean major curve 55.8° ± 13.2°). All participants received only scoliosis-specific exercises using the SEAS method — no surgery or bracing during the treatment period.
Key results after an average of 2 years (range 1–18 years) of consistent exercises:
- 68% of patients improved by at least 5° on Cobb angle measurement.
- The entire group showed a statistically significant average curve reduction of 4.6° ± 5.0° (P < 0.05).
- Only 3% showed any progression — and even in that case, the annual progression rate dropped dramatically.
- Improvements occurred across thoracic, thoracolumbar, and lumbar curves, regardless of age or sex.
The authors concluded that scoliosis-specific exercises are superior to the natural history of adult idiopathic scoliosis and should be considered a first-line conservative treatment, especially for patients who wish to avoid or delay surgery.
Study 2: SEAS — A Modern, Evidence-Based Approach to Scoliosis-Specific Exercises
A companion review article in the same journal detailed the Scientific Exercise Approach to Scoliosis (SEAS), summarizing its neurophysiological foundation and results from multiple prospective cohorts and a randomized controlled trial.
Highlights of the evidence:
- SEAS uses individualized active self-correction in three dimensions (coronal shift to reduce the curve, sagittal restoration of physiological curves, derotation, and axial elongation) followed by stabilizing exercises that challenge balance and function.
- In adolescents with 10–25° curves, SEAS significantly reduced the risk of progression compared to usual physiotherapy or no exercises.
- When combined with bracing, SEAS further lowered progression rates (e.g., 6.1% vs. 25% in controls).
- In adults, SEAS stabilized or improved curves (one referenced cohort showed reduction from 51° to 47° over 3.5 years).
- The program is efficient: short outpatient sessions (45 min, 2–3×/week) plus a simple daily home routine, making it practical for long-term adherence.
The review emphasizes that SEAS breaks the “vicious cycle” of asymmetric loading by training the spine and nervous system to maintain corrected posture automatically — leading to better aesthetics, balance, trunk rotation, and reduced need for more invasive interventions.
